OSX Server 10.4.10 – HTTP Error 401.2 – Unauthorized: Access is denied due to server configuration.

I am getting an intermittent error:
HTTP Error 401.2 – Unauthorized: Access is denied due to server configuration.

When I put in the URL, or even just the IP address, a user name/password box comes up

It is the only site I am hosting on the server and if I start and stop the Web service , the page will display properly without asking for credentials.

I am running Portfolio 8.5 Netpublish on the same server, but the error occurs even when the Netpublish server is stopped – Any ideas why this error is occurring?

What does the web server log have to say about it?

Here is the server log – the problem is occuring right now:

IP addresses are internal, so I am not concerned about any intrusions
I am not sure what the 10.234.9.190 is… that is not the server address

What type of index file are you using? htm, html, php? We had the same problem when trying to use 1 of those 3 that wasn’t default, forget which one. Check your index file type and check the default index types on the general tab wen editing your site in server admin. You could also try typing in the index directly to see if that works.

I was using index.html.
Since this is the only site hosted on the server, I had moved all the default files that are in /library/webserver/documents/ to new folder called “old” and put my new index page there. It is a simple HTML document with one image and one hyperlink.

After viewing your above posting, I decided to create a subfolder for the site within /library/webserver/documents/ instead of it just being at the root level and made sure the server points there and that “index.html” is the selected default.

I restarted the web service and it worked for about 5 minutes – meaning I was able to access the page (from outside of the network, on an entirely different subnet 75 miles away). After around 5 – 7 minutes, it stopped working again – meaning, it is looking for authentication again. Below are the logs since restarting the service:

I have since deleted the default Realm setting and restarted the service with no luck – it constantly asks for authentication after a few minutes… I am totally baffled.
